Midgets	11,12,13 & 14 Yrs. Old

The Registration Process for the 2009 JTAA Cheerleading
You will need:

• Copy of Original Birth Certificate
  
• 4 (four) Copies of Final 2009 Report Card - Front & Back
  
• Copy of Health Insurance Card
  
• Current Wallet Sized Picture
  
• POP WARNER PHYSICAL FORM - Must be completed by a Physician
